Support for ongoing conditions such as diabetes, high blood pressure, asthma, and other long-term health needs.
Medication & Referral Support
Help with medication options, low-cost pharmacy resources, and referrals to specialty care when available.

No. If you are experiencing a medical emergency, call 911.
Southwest Bridge Health uses income information to confirm eligibility for free clinic services. Patients must be uninsured and have a household income below 250% of the Federal Poverty Level. Income information may also help us connect patients with additional support when available.
No. Southwest Bridge Health is for uninsured patients only. We are not able to see patients with Medicare, Medicaid, private insurance, or high-deductible plans.
No. We welcome all neighbors and do not ask immigration status questions.
